FEES and INSURANCE INFORMATION:
RATES:
Depending on services costs can range between
185.00 - 200.00 per session

INSURANCE:
I do not accept insurance and am considered an out-of-network provider. I will however provide billing and a health insurance claim form (with coding) for you to submit to your insurance carrier for potential/partial reimbursement.
You should check your coverage prior to your first appointment by asking your insurance company the following questions:
1. Do I have mental health insurance benefits?
2. What is my deductible and has it been met?
3. How many sessions per year does my health insurance cover?
4. What is the coverage amount per therapy session for an out-of-network provider?
Psychotherapy services may also be reimbursed under your Flex Plan or medical savings account.
PAYMENT OPTIONS:
I accept cash, credit cards, or personal checks. Fees are expected to be collected at the end of each therapy session.
CANCELLATION POLICY:
I require a 24 hour notice of cancellation of your appointment.
If you need to cancel your appointment, please contact me within 24 hours in advance to avoid a full charge.
CONFIDENTIALITY:
The law protects the relationship between a client and a psychotherapist, and information shared in session cannot be disclosed without the written permission of the client.
EXCEPTIONS TO CONFIDENTIALITY INCLUDE:
1. Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report immediately to the appropriate authorities.
2. If a client is threatening serious bodily harm to another person(s), I must notify the police and inform the intended victim.
3. If a client intends to harm himself/herself, I will make every effort to enlist their cooperation in insuring their safety. If they do not cooperate I will take further measures without their permission that are provided to me by the law in order to ensure their safety.
